Lutheran Family Services of Nebraska, Inc. (LFS) takes client privacy very seriously, and it is important to us that you are made fully aware of a potential privacy issue.
We have learned the personal information, including name and date of birth, of certain Community Services clients may have been compromised. On May 3, 2019, a 2014 file with a client list was inadvertently opened by a person no longer affiliated with LFS. The person who opened the file immediately reported the activity to LFS on the same day, May 3, 2019. This file was stored in an unsecured location outside of LFS control between September 12, 2014 and May 3, 2019, when it was discovered and removed.
We have conducted a risk assessment and have taken steps to mitigate any harm to our clients. We provide ongoing training related to information security to our staff and continually evaluate our policies and procedures to ensure client information is protected. LFS is committed to providing quality care, including protecting our clients’ personal information, and we want to assure you we have policies and procedures in place to protect client privacy.
While we do not have any indication the information was used inappropriately, it is important our Community Services clients are aware of the breach so they may monitor their credit reports for any unusual activity. We understand this may pose an inconvenience. We sincerely apologize and regret this situation has occurred, and are working diligently to ensure the privacy and security of all personal client information.
